What You'll Need
We make enrollment simple. To complete your child's enrollment, please have the following:
- Completed Enrollment Packet
- Child's Birth Certificate
- Current Immunization Record
- Physical Examination Form
- Parent/Guardian Photo ID
- Authorized Pick-Up List
- Child Care Subsidy Information (if applicable)
- Private Pre-K Tuition Agreement (Private Pre-K f-milies only)
- Registration Fee (if applicable)
